The North Greenville University Sports Medicine Department's mission is to provide exceptional care and support to our student-athletes by delivering evidence-based therapeutic care and comprehensive rehabilitation services to prevent and care for injuries; while promoting their overall health, well-being, and optimal athletic performance.

The following documentation must be completed by each student-athlete prior to participation in NGU intercollegiate athletics. If paperwork is not submitted in full by the deadline, the student-athlete will not be permitted to participate.

  • All paperwork is completed annually in accordance with NCAA requirements.
  • Going forward, most forms will be completed independently by the student-athlete before leaving campus for the summer.
  • We strongly encourage parents and guardians to complete this paperwork together with the student-athlete the first time, so the athlete is familiar with the process in future years.
  • Forms requiring a physician’s signature must be uploaded separately and will not be accepted unless complete in their entirety.

 FAQs 

1. Where can I find the paperwork to get started? A: If you are officially on the roster, you should have an email containing the invitation to get started on Teamworks. Once you fill out the NGU Student Athlete Demographic Sheet on Teamworks, that information will be used to create your Healthy Roster account. That link will also be sent to your email. 

2. Do I need a physical before I come to campus? A: You do NOT need a physical before coming to campus. All Freshmen and Transfers will do a physical with our physicians upon arrival to campus, before the first official team activity. Further details on date and time will be provided by your head coach.  

3. Are all insurance plans accepted as primary insurance? A: No. We do not accept Medicare, Medicaid, Tri-care, or Christian Ministry plans as primary insurance plans. There are some few exceptions (See Student-Athlete Insurance Form on Teamworks). 

4. What are my options if I do not have a valid, active primary insurance plan?  We have an accident insurance policy that meets the minimum requirement that you can purchase. You can also enroll in a different insurance plan on your own that meets the minimum requirements. Contact your team’s athletic trainer for more information on both. 

5. Where can I do a sickle cell test? You can contact your nearest LabCorp and provide them with the correct requisition form (more details on NGU Sports Medicine Webpage). You may contact your primary care physician to set up the test and access those results. Make sure the correct test is used, either sickle cell solubility to electrophoresis.
